COMMITMENT PERIOD AGREEMENT

This Commitment Period Agreement (“Agreement”) is entered into between Dialog Axiata PLC ("Dialog") bearing company registration number PQ 38 and having its registered office at No. 475, Union Place, Colombo 02 and the Customer (“I”) in relation to the subscription of a commitment-based package and the associated Air Fibre device provided under the subscription.

This Agreement is supplemental to and shall be read in conjunction with the Dialog Air Fibre Terms and Conditions published at https://dialog.lk/tc/postpaid-application-form-mobile. In the case of any inconsistency between this Agreement and the Dialog Air Fibre Terms and Conditions, this Agreement shall prevail to the extent of the matters covered herein.

• I hereby confirm that I have subscribed to one of the packages listed below, as specified in my application form, and agree to be bound by the applicable commitment period.
• I understand and agree that the Commitment Period will commence from the date of service activation (“Commencement Date”).
• I acknowledge that the Air Fibre device issued to me is provided as part of a commitment-based offering and is linked to the subscribed package throughout the Commitment Period.
• During the Commitment Period:

  4.1 Package upgrades may be permitted in accordance with Dialog's prevailing policies.

  4.2 Package downgrades are not permitted.

  4.3 Disconnection, termination, or cancellation of the connection is not permitted without applicable charges.

  4.4 Conversion of the connection from postpaid to prepaid is not permitted.

• I understand that the Air Fibre device and the subscribed package are provided together under a commitment arrangement. Any promotional pricing, discounts, subsidies, or benefits offered on the device and/or package are conditional upon fulfilling the Commitment Period.
• In the event of early termination, cancellation, or breach of the commitment conditions, Dialog reserves the right to recover any applicable Early Termination Fee (ETF), device subsidy, outstanding charges, or any other applicable fees in accordance with Dialog's prevailing policies.
• I agree to comply with Dialog's applicable Terms and Conditions, service policies, and any amendments communicated by Dialog from time to time.

I hereby acknowledge and confirm that I have read, fully understood, and agree to be bound by the Terms and Conditions of the Commitment Period Agreement.
