3. Why do I need an Address confirmation document?

If your address is different/unavailable on your ID document, you will be required to upload any of the below-listed documents which have your current billing address details.


Examples of instances requiring an address confirmation document

>  Passport is given as the identity document

>  Driving License does not have an address

>  Address not clear on NIC/Driving License

>  If present/correspondence address is different from the ID document


Note

>  The name on the address confirmation document should match the name on the NIC/Driving License/Passport

>  Refer to the acceptable validity period for the address confirmation document below


Document list:


>  Currently active Dialog Postpaid bill for Mobile, Home Broadband, CDMA or Dialog Television

>  Utility bill payments (one bill within last 6 months)

>  Bank statements from the last 2 months (e-statements will not be accepted)

>  Documents received from banks (reminders/card-related/loan-related/pawning-related) - issued by the bank within the last 2 months

>  Credit card statements (validity: last 2 months)

>  Letter from a government institution on the letterhead

>  GS certificate

>  Tax payee invoice received by post with address (validity: last 6 months)

>  Other service provider bills - fixed lines only within the last 6 months

>  Election cards issued within the last 6 months

>  Religious leaders’ identity card address

>  Government service ID address (doctors/lawyers/forces/teachers) - must be a valid ID

>  Registered lease agreements (land/ property or vehicle)

>  Government service ID

>  Letters received from Courts during the last 3 months

>  Government Pensioner’s ID/Government Elder’s ID
